Line of Inquiry: How did the failure to apply Pareto's Principle affect the consequences of this event? Are you sure that it didn't happen because Pareto's Principle was being applied (if even subconsciously) ? This event seems to be a much better illustration of the Law of Multiple Prevention than of Pareto's Principle. Your pairing of these two hypotheses (seemingly a better term than either "law" or "rule") would seem to imply that they are somehow interrelated -- an extrapolation with which I would not necessarily concur as neither provides a necessary and sufficient set of conditions for the other. Suggestion: Treat these as separate rather than as pseudo-related hypotheses.

Plus, use of numerical percentages (e.g., 80%, 20%) consistently throughout the text would conserve space and enhance appearance.

Multiple Prevention, Law of (See "Pareto's Principle.")

For every consequence there are multiple factors, which if they had not existed as they were, the consequence would have been prevented or greatly mitigated.

It's plain English and it's true.

Line of Inquiry; What are the factors, which if they had not existed as they were, the consequence would have been prevented or greatly mitigated?

Tools: 1) Factors-consequence s Matrix (FCM), Comparative timelIne (CTL), Corrective Action Matrix (CAM)...

Pareto's Principle(aka "The 80-20 Rule" of words to that effect)

"...for many events, roughly 80% of the effects come from 20% of the causes."

It's the way it's usually used.

It needs to be understood in light of the "Law of Multiple Prevention", which see.

Example: Eighty per cent of rework was associated with twenty per cent of the work packages.

Example: Eight per cent of special inspections were at twenty percent of the facilities.
